- $150 Non-refundable application fee (required to start-processing fee applies)
- $32.89 Non-refundable student license fee

We do not accept Financial Aide or VA benefits at this time.
Students are required to wear solid black scrubs (top/bottom-no logos).
Students are responsible for the testing fees for the state board written and practical examinations once they have completed the program.

I understand that once I have been notified by the school that my registration application has been reviewed/approved I will need to complete an enrollment packet and submit my enrollment fee ($1289.89) in order to secure my seat, cover my books, kit & supplies.

I understand that I must submit the $150 application fee in order to complete my registration.

Tax Compliance

All persons and business entities licensed with the Division of Professional Registration are required to have paid all state taxes and also are required to have filed all necessary state income tax returns for the preceding three years. If you have failed to pay your taxes or have failed to file your tax returns, your license will be subject to suspension within 90 days of being notified by the Missouri Department of Revenue of any delinquency or failure to file. The following tax information must be provided.

2x2 Photo: Required for admission. Students may provide their best photo/selfie with a solid background (preferably white) from the neck up.
